Dear School Community:

Due to high interest among students, staff and their families, the Westmoreland Central School District is holding a COVID-19 Vaccination Clinic on Thursday, May 27, from 10:00 a.m.-1:00 p.m., in the Jr./Sr. High School Gym. An appointment time is not required.

The clinic is being offered by the Upstate Family Health Center, Inc. in collaboration with the Oneida County Department of Health. The Pfizer vaccine, which has been approved for individuals ages 12 and older, will be administered. Pfizer is given in two does, three weeks apart.

Students under the age of 18 who would like to receive the Pfizer vaccine must have a consent form signed by a parent or guardian. Students must present the signed consent form at the time of vaccination. If necessary, a consent form can be signed by a parent or guardian the day of the vaccination clinic or, as a last resort, consent can be granted over the phone. Students under the age of 18 without parent/guardian consent will not be vaccinated. Click here to download, print and sign a consent form.

The vaccine is free, but Upstate Family Health Center, Inc. may charge for the administration of the vaccine. However, there is no out-of-pocket cost for those who do not have health insurance. The insurance section on the consent form may be left blank.

To view a “fact sheet” on the Pfizer vaccine, click here. Please read the “fact sheet” before receiving the vaccine.

For all of those who receive the first dose, second doses of the Pfizer vaccine will be administered on Thursday, June 17, here at Westmoreland. More details will be announced at a later date.

*If you are 12 or older and would like to receive the Pfizer vaccine at Westmoreland, please let us know by completing a one question survey by Tuesday, May 25. Click here to complete the survey.*

Should you have any questions about the Pfizer vaccine or its administration, please contact Richard Williams of the Upstate Family Health Center, Inc. at (315) 624-9473 or Richard.Williams@ufhcinc.org.

Sincerely,

Rocco Migliori
