Important Information
Physicals
New York State law requires all students entering Pre-K, Kindergarten and First grade, as well as any student entering the district for the first time, to have a physical.
If your child has had a physical within the last year, please fax a copy to (315) 853-6597, Attn. Alicia Brown R.N. Any physical dated 9/4/21 or after will be accepted. Click here to download a "Required NYS School Health Examination Form."
If your child has an upcoming physical scheduled during the summer or fall, please inform the Primary Elementary School health office of the appointment date. If your child has not had a physical within the last year, he or she can receive a physical exam at no cost from a nurse practitioner while school is in session. If you wish to schedule a physical during normal school hours, please contact the Primary Elementary School health office.
Vaccines
New York State Law Section 2164 requires certain immunizations (shots) to enter Kindergarten and attend school. Click here to view a list of required immunizations. Contact your health care provider as soon as possible to verify the need of any required vaccine(s). If you do not have a health care provider, you can contact the Oneida County Health Department at (315) 798-5747 to schedule an immunization appointment.
Hearing and Vision Screenings
Students in Pre-K, Kindergarten and First grade, as well as newly enrolled students, who do not have a hearing and/or vision screening noted on their health certificate or health appraisal, will be screened.
